All donations made through St. Jude Neighborhood Health Centers website donation portal are made through Farmers and Merchants Bank, a Donor Advised Fund (“DAF”) recognized by the Internal Revenue Service (“IRS”) under Section 501(c)(3) of the Code as a tax-exempt public charity (Federal Tax ID: 45-3977605) Health Centers donation portal, Donors acknowledge that their donations will be granted to recommended services and goods pursuant to such Donor Terms of Service. Donors will receive a date-stamped receipt of donation from St. Jude Neighborhood Health Centers marking the date the donation was made.

This policy establishes notice and consent requirements expected of individuals, third parties, and service providers as it pertains to their specific relationship to the personal data in question. An individual who donates to St. Jude Neighborhood Health Centers enters willingly into this agreement. Information you send or get through the site is private. St. Jude Neighborhood Health Centers does not share your information, and we do not sell or license your information. Collection or processing of personal data of an individual will only be made when the individual has consented for a specific purpose or in accordance with a permissible purpose as described in this policy.

When you donate through our portal, you will need to enter your name, email address, phone number and address listed. You will be required to manually enter your credit card details. This information allows us to process and fulfill your donation successfully. When entering you donation details, we use a secure server. Furthermore, all the sensitive and personal data that is entered is protected by several layers of encryption and several layers of security to prevent unauthorized access.

Payment by credit or debit card is the only way to donate. All credit or debit card donations are processed securely to Farmers and Merchants Bank donations account. St. Jude Neighborhood Health Centers does not store any credit card information on our servers. All uses and processes of your donation information is in accordance with our privacy standards. Credit Card donations are made directly through a 256-bit SSL Encryption software to ensure your personal details are kept safe.

By using our website and donation portal, you consent to the use of the information entered to process said donation. None of your credit card details are stored. This ensures you will be satisfied in the security of your donation.

Need-specific costs or donations in general, are not a promotion. In other words, when you donate for an example of a specific need for our health centers, you will not redeem that promotion. You will not be subject to fees which do not apply to someone not redeeming a promotion.

Your donations are deemed valid without unspecified additional user requirements. Need specific donations will not impose requirements to sign up for a paid membership, taking part in a survey to make a donation, and/or you will not receive a discount on goods or services for those solely outlined as examples of our health center needs. There is no promotion period or while supplies last, all donations are welcome through our portal.
